Missed Appointments:

 

    • No-shows or missed appointments without prior notice are not eligible for refunds or rescheduling under the standard policy.

We reserve the right to refuse future bookings in the event of repeated missed appointments or abuse of our scheduling system.

  • While homoeopathy is a complementary therapy recognised and used by many individuals worldwide, its efficacy may vary from person to person. Results are assured but not guaranteed and may differ depending on individual health profiles and lifestyle factors.

 

  • EVYA Healthcare’s Homoeopathic consultations are not intended to replace conventional medical treatment. In cases of serious, chronic, or life-threatening conditions, we strongly recommend consulting your primary healthcare provider or a specialist. If you have a health condition or are currently receiving medical treatment, you should continue to consult your GP or specialist.

 

  • EVYA Healthcare practitioners are registered and trained in accordance with applicable regulations. However, homoeopathy is not a replacement for emergency medical care or hospital services.

 

  • By engaging with our services, you acknowledge and agree to the above and take full responsibility for your healthcare choices.

Consent and Agreement that Person will digitally sign.

 

  1. I voluntarily consent to receive treatment from Doctor Ashish at Evya Healthcare Limited through teleconsultation.

    The doctor has thoroughly explained the nature and method of the treatment, including any diagnostic procedures (whether involving equipment or otherwise), and has satisfactorily addressed all my questions regarding my condition and the proposed treatment.

 

  1. I have reviewed the treatment plan and associated charges, and I agree to pay the stated fees. I understand and accept that these charges are non-refundable and non-adjustable.

 

  1. I acknowledge that the doctor has made no guarantees regarding the outcome or duration of the treatment.

 

  1. I agree that the doctor shall not be held liable under any legal theory or indemnity in connection with my treatment. I hereby release and permanently waive any claims I may have against the doctor related to this treatment.

 

  1. For the purpose of helping and educating others, I have no objection to the observation or photography of my treatment, including the publication of before-and-after images showing improvements in my condition. This is permitted only if my identity is not disclosed through the images or accompanying descriptions.

 

  1. I agree to provide accurate information about my medical history and any known allergies. I also commit to cooperating fully with the doctor and following their instructions and recommendations to the best of my ability.

 

  1. I digitally sign this registration form and retain a copy as confirmation of my agreement to the terms outlined herein, applicable to registration, extension, and renewal.

 

  1. I consent to receive promotional emails and SMS communications from Evya Healthcare Limited.
